For new patients, please download, print out, and complete our Office Information Forms and HIPAA Policy prior to your visit. This will speed your registration process. You can bring the completed form with you to your appointment; or, if you're digitally savvy, complete the form with your PDF software and email it to info@alexorthop.com ahead of time. Be sure to encrypt the form before you click "send" to maintain privacy issues. For hip, knee, and leg conditions, please bring a pair of shorts for your exam. For shoulder conditions, ladies should please wear a "sports bra" for your exam. |
